The search strategies for PEARLS (on the left column) was taken from the book 'Evidence-Based Medicine USydMP HANDBOOK' 5th Edition 2007

Search strategy for PEARLS (2007)

Alternative search suggested by KM 2009

Search A  - EBM Reviews in CDSR

(a) Limit to systematic Reviews

(b) Only useful for questions about treatment

Search A-KM – ALL EBM Reviews in OVID

(a) Do not limit to SR first -(What are you searching for when you search 'ALL EBM ERVIEWS' in OVID?)

(b) Even the CDSR now include articles about diagnostic tests

Search B

Comprehensive search of Medline (Ovid version) using MeSH, then applying the Reviews limit

Search B & C

I would suggest that instead of B and then C you try a PubMed search (MEDLINE) using MeSH and filtering using Clinical Queries in one go.

Search C

If search B did not produce results then limit your comprehensive Medline search with Medline Clinical Queries Filter

Search D

Search PubMed Clinical Queries. This is an alternative to search C, however it requires changing databases

Important Note:

Apart from Search A, which looks at CDSR, the other search strategies involve filters that will look for the best PRIMARY STUDY TYPE to answer the question. However, in all cases a systematic review(SR) that combines multiple primary studies is always better than the stated best primary study stype. If you do not find any systematic reviews in Cochrane (Also rememberCochrane only has systematic reviews of intervention studies), you can look for a systematic review in Medline by doing the following:

IMPORTANT NOTE - KM

In all cases a SR will not always be better than a primary studies. A normal occurrence is that a recent primary study (e.g. RCT with large sample size) may not be added to an existing Cochran SR for a number of months or years - this is a problem for CDSR because most of the work is done on a voluntary basis). Therefore it is better to search for SR plus recent primary studies in one search (PubMed search using MeSH) than only searching CDSR (which only searches a small number of the total SR available in health care). Note that PubMed includes all CDSR with only a minimal delay after they appear in the CDSR database.

Cocharne SR now include SR for intervention plus diagnostic studies
